District Nurses

The district nursing team is led by an experienced qualified district nurse. The team is dedicated to looking after patients within their own homes, when they are unable to attend surgery.

District Nurses are available 24 hours a day, seven days a week.


  • Wound management
  • Pain management
  • Palliative care
  • Assessment of care needs
  • Diabetic monitoring
  • Blood tests
  • Continence management
  • Flu vaccinations (housebound only)

Wound Management

  • Assessment and treatment of all types of wounds.
  • Surgical, traumatic, pressure damage, leg ulcers.
  • Regular weekly leg ulcer clinic held at Bridgwater Hospital where full assessment and treatment provided.
  • Referral to the ulcer clinic is via GP, practice nurse or district nurse.

Palliative Care

  • To provide care and support to patients and carers to enable them to maintain a quality of life.
  • To provide information and access to other services available, i.e. Hospice, Marie Curie nurses.
  • Assessment of Care Needs

The district nurse works in conjunction with the patient, carer and Social Services to determine the level of care needed and if required, placement in a Residential/Nursing Home.

Continence Management

To help manage continence problems and provide the best care plan, an individual assessment will be carried out by the district nursing team.
